Just look at the person next to you and pass the blame. Or giggle. That's what I usually do

Depends on the situation. In a formal or business situation, where it was impossible for your gaffe to go unnoticed? Yes, say something. Otherwise, do whatever's comfortable. Pass the blame, ignore it, or whatever.
